Custom Home Care (CHC) is seeking a Nurse Care Manager to join our growing team. In this role, you’ll provide wellness oversight, thoughtful guidance, and practical decision‑making support to older adults and their families. Consistent Weekday Schedule — Set Monday–Friday hours with no weekends or holidays

Key Responsibilities

Conduct client assessments — Complete assessments for new clients and develop individualized care plans for hourly and live‑in services
Perform routine visits — Monitor ongoing conditions, reassess as needed, and update care plans
Educate and advise families — Provide clear guidance on care needs and recommendations
Communicate and oversee care plans — Share care plans with CHC Care Professionals and offer ongoing coaching
Provide performance feedback — Deliver constructive evaluations and training for Care Professionals
Coordinate care — Facilitate communication to ensure continuity of care
Report health status changes — Keep community management informed of changes in client health
Maintain referral relationships — Build and sustain strong partnerships with referral sources
Engage potential clients — Identify and connect with individuals who may benefit from home care
Use technology effectively — Document care and communicate efficiently using CHC’s digital platforms

Skills & Competencies

Geriatric nursing expertise — Skilled in assessments, care planning, problem‑solving, and advising families
Supervisory and leadership ability — Experience overseeing, coaching, and supporting Care Professionals
Relationship building and interpersonal skills — Strong ability to engage clients, foster trust, and maintain meaningful relationships; enjoys helping others in a compassionate, client‑centered way
Clear communication — Excellent verbal and written communication with clients, families, referral sources, and health professionals
Technology proficiency — Comfortable using digital tools and learning new systems
Team and independent work — Able to work independently and collaboratively within a team of 10+ Nurse Care Managers
Time management and organization — Exceptional ability to prioritize and manage multiple responsibilities
Interpersonal and decision‑making strengths — Strong customer service, problem‑solving, and sound nursing judgment

Education & Experience

Registered Nurse (RN) with a current Illinois license
Experience working with the geriatric population, including individuals living with dementia
