COMPANY SUMMARY

The Center for Comprehensive Health Practice (CCHP) is a multi-disciplinary community health center and substance abuse program located in East Harlem New York. Accredited by CARF (the Commission on Accreditation of Rehabilitation Facilities) and supported by OASAS (the Office of Alcoholism and Substance Abuse Services). CCHP serves the most vulnerable communities in New York City, primarily East Harlem and the South Bronx but patients come from all five boroughs of New York City.

We are currently seeking a Re-entry Recovery Specialist to assist in one of our programs and lend value to the CCHP team.

JOB DESCRIPTION

The Recovery Specialist role is to support others in recovery from a substance use disorder,

The ideal candidate will use their personal experience with addiction recovery and reentry to provide guidance, support, and mentorship to individuals currently navigating similar challenges. This role is crucial in helping clients build resilience, develop coping strategies, and foster a sense of hope and empowerment. The Recovery Specialist will serve as a role model, mentor, advocate and monitor to recovering individuals in order to prevent relapse and promote log-term recovery

KEY RESPONSIBILITIES

 Serve as a trusted mentor by sharing lived experience with incarceration, reentry, and recovery to build rapport and inspire hope.

 Conduct outreach in jails, reentry centers, parole/probation offices, harm reduction programs, and other community settings across NYC.

 Facilitate peer-led support groups and workshops focused on reentry and SUD-related topics such as housing, trauma recovery, employment, relapse prevention, and building sober support networks.

 Assist clients in accessing critical reentry and recovery resources, including housing, employment, education, mental health and substance use treatment, medical care, public benefits (e.g., SNAP, Medicaid, HEAP), and legal documentation.

 Provide peer-based navigation for clients involved in the criminal legal system, including support with parole/probation requirements, court mandates, and reentry planning; accompany clients to appointments when needed.

 Participate in weekly supervision and interdisciplinary case conferences with counselors, medical providers, and psychiatrists to ensure coordinated, holistic care for clients with justice-involvement and SUD.

 Deliver person-centered assessments, service plans, and recovery support services that reflect both reentry goals and substance use treatment needs.

 Offer individual peer counseling focused on coping skills, wellness strategies, harm reduction, and community reintegration.

 Conduct home visits or community-based check-ins as needed to maintain engagement and escort clients to essential services.

 Develop and sustain partnerships with NYC-based organizations that support reentry and recovery to ensure effective referrals and warm handoffs.

 Maintain timely, accurate, and confidential documentation of client interactions, goals, and progress in compliance with agency and funder requirements.

 Actively engage in training on trauma-informed care, harm reduction, recovery principles, reentry support, and professional boundaries.

 Perform other duties as assigned by the supervisor or program leadership.

QUALIFICATIONS

 Lived experience with incarceration and reentry in NYC.

 Strong understanding of the challenges associated with addiction and reentry, and the ability to

relate to patients on a personal level.

 Must have high school diploma or equivalency and CPRA or CRPA-T

 Minimum of 2 years recovery specialist/peer advocate experience

 Bilingual in Spanish and English preferred

 Strong interpersonal skills, empathy, and the ability to set and maintain healthy boundaries.

 Working knowledge of NYC’s public systems, reentry resources, and community-based supports.

 Experience in peer advocacy, group facilitation, or case navigation preferred.

 Comfortable with documentation, communication, and use of electronic databases.

 Ability to travel across NYC’s boroughs; flexible scheduling required.
