Job Summary
We are seeking a dedicated and detail-oriented Medical Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate and oversee patient care plans, ensuring seamless communication among healthcare providers, patients, and families. Your expertise will help optimize patient outcomes, facilitate discharge planning, and manage complex cases across various medical settings. This position offers an exciting opportunity to make a meaningful impact by providing compassionate, comprehensive case management services in a fast-paced healthcare environment.

Duties

Conduct thorough patient intake assessments to determine individual needs and develop personalized care plans.
Coordinate multidisciplinary care teams including social workers, nurses, physicians, and specialists to ensure cohesive patient management.
Oversee discharge planning processes, ensuring timely and appropriate transfer of patients from hospital to home or other care facilities.
Utilize EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems to document patient interactions, treatment plans, and progress accurately.
Perform utilization review and management to ensure appropriate resource use while complying with HIPAA regulations.
Apply clinical expertise in areas such as critical care, ICU, emergency medicine, pediatrics, and hospice care to support complex cases.
Manage case documentation involving CPT coding, ICD-9/10 coding systems for billing and reporting purposes.
Collaborate with hospital staff experienced in trauma centers (Level I & II), PICU (Pediatric Intensive Care Unit), and acute care settings to facilitate optimal patient outcomes.
Provide education on medical terminology, discharge instructions, and behavioral health considerations to patients and families.

Preferred Qualifications

Proven experience in hospital settings with critical care, ICU, emergency medicine or trauma center environments preferred.
Strong nursing background with active licensure; experience in pediatrics or hospice care is a plus.
Experience working in HIV/AIDS services, public health, healthcare, or a related field.
Working knowledge of the Ryan White HIV/AIDS Program, including program requirements, documentation, and compliance standards is a plus.
Familiarity with HIV prevention, treatment, and support services.
Knowledge of CPT coding, ICD-9/10 coding systems, ICD coding standards for accurate documentation and billing.
Familiarity with EMR/EHR systems and utilization management protocols.
Excellent understanding of HIPAA regulations related to patient privacy and data security.
Experience in discharge planning, intake procedures, and managed care processes.
Ability to work effectively within multidisciplinary teams including social work and behavioral health professionals.
Strong organizational skills with attention to detail in case documentation and reporting.
